UnitedHealthcare rehab coverage falls into three main categories: health insurance plans, Medicare plans, and Medicaid plans, which serve different medical needs. Below, we take a look at the different plans and what they involve.

UnitedHealthcare HMO Plans

A health maintenance organization (HMO) plan keeps your care organized through a network of doctors, clinics, and hospitals. If you have this type of plan, you will choose a primary care doctor who then manages your healthcare with other doctors in-network. If you need to see a psychiatrist or a rehab specialist, you will need a referral from your primary care doctor first.

To use the full benefits of the plan, you would have to receive treatment from providers who are part of the HMO network. If a rehab center or therapist is outside that network, the plan would not cover the rehab services at all, except in emergencies.

HMO plans have lower monthly premiums and predictable costs because your care stays inside a defined network of healthcare providers.

UnitedHealthcare PPO Plans

A preferred provider organization (PPO) plan is much more flexible than an HMO. Like an HMO, it also has a network of doctors, therapists, hospitals, and treatment facilities. But in addition, you are also allowed to visit providers outside the network.

Obviously, going out of network will cost you more, but you won’t have to bear all of it. Staying in-network will result in a larger portion of the cost being covered by the plan. But if you go outside the network, the coverage will still apply at higher out-of-pocket expenses.

You also do not need a referral to see a specialist under a PPO plan. If you want to seek therapy, psychiatric care, or addiction treatment, you can contact a rehab center directly and schedule an evaluation.

UnitedHealthcare POS Plans

A point of service (POS) plan gives you the best of both worlds. In other words, it combines features from HMO and PPO plans.

Like an HMO plan, you need to select a primary care provider who can coordinate your healthcare services. That doctor will also provide you with referrals when specialized treatment is needed.

But you can also go out-of-network if you want, at higher costs. If your care stays inside the network, the plan will cover a larger share of the expense.

UnitedHealthcare Choice Plus

The UnitedHealthcare Choice Plus plan is one of the more flexible health insurance options. It gives you a lot of choice when it comes to doctors, specialists, and treatment centers.

You do not need a referral to see a specialist, either. You can schedule an appointment with a therapist, psychiatrist, or other specialist directly, without first visiting a primary care doctor, whenever you want.

Similar to a PPO plan, if you decide to see a doctor who is not part of the network, the plan will still cover some of the costs. But the amount you pay yourself will generally be higher.

Preventive rehab through Choice Plus plans, which includes annual checkups and screenings, is covered at 100% when performed at an in-network facility.

Optum Behavioral Health Plans

You can also receive your mental health and addiction treatment benefits through Optum Behavioral Health. Optum is a healthcare services company that operates under UnitedHealth Group, specifically to manage behavioral health services for UnitedHealthcare insurance plans.

In simple terms, UnitedHealthcare will provide you with the insurance coverage. But the management of the mental health and rehab portion of it, like therapy networks or treatment approvals, will be done under Optum.

Here are the services Optum covers:

    • Individual therapy and counseling
    • Family or couples counseling
    • Psychiatric evaluations
    • Medication management with a psychiatrist
    • Psychological assessments and diagnostic testing

What Does UnitedHealthcare Cover for Addiction Treatment?

Addiction is recognized as a medical condition. Therefore, federal law requires health insurance providers to treat substance use disorders the same way they treat other medical conditions.

So, UnitedHealthcare must provide coverage for mental health and addiction treatment that is comparable to the coverage offered for physical health conditions under the Mental Health Parity and Addiction Equity Act.2

Below, we explain the services that insurance should cover as part of this treatment.

Detox and Medical Services

The first step to treatment for substance use disorders is medical detoxification. In this process, the body is prepped to safely clear drugs or alcohol, and healthcare professionals manage withdrawal symptoms alongside.

All UnitedHealthcare plans cover medically supervised detox programs for alcohol, benzodiazepines, and opioids. Your coverage includes withdrawal management services through which the medical team will monitor your vital signs and manage your symptoms during detox.

Many UnitedHealthcare plans also cover medication-assisted treatment (MAT).3 MAT is an FDA-approved intervention to combine medications with counseling and behavioral therapy. It reduces substance misuse relapse. Medicines prescribed for MAT include buprenorphine, methadone, and naltrexone.

Residential Treatment

UnitedHealthcare plans will cover residential addiction treatment when it is deemed medically necessary.

Residential programs allow people to live at a treatment facility where they receive inpatient rehabilitation services. They operate with 24-hour clinical supervision of trained staff and medical professionals.

Therapy, both individual and group-based, is a central component of residential treatment. Many treatment programs also include family therapy.

Mental Health Treatment

About 21.2 million adults in the United States have both a substance use disorder and a mental health problem at the same time.4 This is known as “dual diagnosis.”

All UnitedHealthcare plans cover dual diagnosis treatment, which addresses both substance use disorders and co-occurring mental health conditions.

Your coverage will include treatment for:

Optum Behavioral Health Coverage

Many people with UnitedHealthcare insurance receive their rehab benefits through Optum Behavioral Health. Optum serves as the behavioral health division within UnitedHealthcare Group. Therefore, it manages mental health and substance use treatment services.

You may see “Optum” or “Optum Behavioral Health” printed on your UnitedHealthcare membership card. If you do, you can contact your treatment provider with their Optum member ID.

Before you begin rehab treatment, Optum will require confirmation that the recommended level of care is medically appropriate for you. Your treatment providers will communicate it for you. The clinical team will share the details of the recommended treatment plan for authorization and utilization review.

To verify your insurance, we will need a few details from your UnitedHealthcare policy that can help us locate your plan. Please have the following information ready when you reach out to us:

    • Your UnitedHealthcare or Optum member ID number
    • Your group number (if your plan is provided through an employer)
    • The policyholder’s name
    • The policyholder’s date of birth
    • Your plan type, such as PPO, HMO, or Choice Plus

    Most of this information will simply be written on the front of your insurance card.

Once we have your insurance details, our admissions team will contact UnitedHealthcare to review your benefits. In most cases, insurance verification takes about 15–30 minutes.

During this time, we confirm whether or not your plan covers addiction or mental health treatment. We will ask UnitedHealthcare if your plan qualifies for higher levels of care and whether your desired treatment programs are in-network or out-of-network with your plan.

Understanding Your UnitedHealthcare Benefits

Navigating the verification process can be difficult enough without having to understand the jargon that often accompanies it. Below are some important terms you should understand to make the most out of your UnitedHealthcare insurance plans.

In-Network vs. Out-of-Network

“In-network” vs “out-of-network” are terms used in insurance policies to describe the relationship between the insurance company and the healthcare provider.

An in-network provider is a doctor or a treatment facility with a formal agreement with UnitedHealthcare. They provide services to patients with UnitedHealthcare insurance plans at pre-negotiated rates.

The insurance company has already established pricing with these providers, so patients have to pay lower out-of-pocket costs when receiving care within the network. You could say that it makes your rehab care more affordable.

An out-of-network provider does not have a contracted rate with UnitedHealthcare. Some UnitedHealthcare PPO and Choice Plus plans include out-of-network benefits for times when you can’t find a good provider within the network.

Your insurance plan will still contribute toward the cost of treatment even if the provider is not part of the network, but a larger share will be your responsibility.

Common UHC Terms

UnitedHealthcare insurance policies will use some words commonly. If you understand these basic definitions, you’ll be better able to interpret your benefits. These include:

  • “Deductible”: The amount you must pay for covered healthcare services before your UnitedHealthcare plan begins sharing the cost.
  • “Copay”: A fixed amount you pay for certain medical services, for example, a plan may require a $25 or $50 copay for a doctor’s visit.
  • “Coinsurance”: The percentage of the cost you pay after your deductible has been met. For example, if your plan has 20% coinsurance, you will pay 20% of the cost, and UHC will cover the remaining 80% of it
  • “Out-of-pocket maximum”: The highest amount you are required to pay for covered services during a plan year. After you reach this maximum, UHC will cover 100% of additional covered services for the rest of the year.

No. All your treatment information, including your insurance details, is protected by HIPAA and federal confidentiality laws (42 CFR Part 2). The insurance claims will show that you received behavioral health services, but their details will not be shared with your employer.
